Chronic Disease is a long-term condition that can be managed but not cured. Chronic diseases – such as heart disease, stroke, cancer, diabetes, and arthritis – are among the most common, costly, and preventable of all health problems in the U.S. Older adults have a high frequency of multiple chronic conditions, which require a more complex care plan. This course provides primary care professionals with an overview of chronic illnesses as well as tools for more effective assessment and management of older patients.
