This webinar will introduce participants to LifeCourse™, a systemic care approach implemented at Allina Health System that involves lay heath workers, called care guides. Learn how these workers are trained to understand needs, leverage strengths and empower families to effectively support their loved ones. Ms. Schellenger will describe the role of care guides and how their activities align with palliative care domains and practices. Explore early findings from Allina’s research on the role of LifeCourse in helping individuals access resources and maintain a better quality of life.
