The purpose of this knowledge based virtual training series is to improve the geriatric (polypharmacy, medication management, dementia diagnosis management, end of life), and musculoskeletal treatment of Veterans in rural areas. At the end of this training, providers will be able to identify the tools in the Computerized Patient Record System (CPRS) environment to document patient goals of care and life sustaining treatment decisions. Members of multidisciplinary teams will understand their roles in the processes of completion and documentation of goals of care conversations. (Part II)
